A CSB safety video about the April 2018 explosion and fire at the Husky Superior Refinery in Superior, Wisconsin. The incident injured 36 workers, caused roughly $550 million in damage to the facility, and released 39,000 pounds of flammable hydrocarbon vapor into the air.

The U.S. Chemical Safety Board (CSB) is an independent, nonregulatory federal agency that investigates the root causes of major chemical incidents.

Our mission is to drive chemical safety change through independent investigations to protect people and the environment. The agency was created under the Clean Air Act Amendments of 1990.

IN THE CSB’S 25-YEAR HISTORY, the agency has deployed to over 170 chemical incidents and issued 1000 recommendations that have led to numerous safety improvements across a wide variety of industries.

BP Husky Chemical Release and Fire 

View information on the CSB's final report into a  September 20, 2022, accidental release of flammable chemical chemicals ignited, creating a fire that fatally injured two workers and resulted in substantial property damages at the BP-Husky refinery in Oregon, Ohio.
